Session 5: Acurata Patient Experience Portal (APEx)

Today is our last session in the Acurata 101 series and we couldn't be more excited to share APEx with you! A true patient and caregiver experience, APEx provides a single pane of glass for your patients and their families. In this series we will learn how patients can track their nurse in real time, request visits, communicate with call center staff, and browse a customized knowledgebase, all from one portal.

 
 

The current trend in healthcare is to allow and encourage patients to be more active participants in their care continuum. APEx makes this incredibly easy.

 

COMMUNICATION

The primary goal of APEx is to simplify communication between patients and caregivers with the provider. Not only does this put a focus on involvement, it also alleviates anxiety from the triage process. Patients and caregivers have full access to basic knowledge that they may otherwise not, or have taken for granted; who their nurse is, where they are, and when they are scheduled to arrive.

In addition to working with hospice and home health organizations, every member of the Acurata team has been a caregiver for family and/or friends in hospice care. We understand what it means to request a triage visit in the middle of the night for a loved one in distress and then anxiously await the arrival of a nurse. APEx helps reduce this anxiety by providing a visual level of comfort and clear lines of communication that are easy to access and use.

 

UNIFYING THE CARE CONTINUUM

The caregiver population is getting more tech-savvy. According to a recent study, 48% of caregivers are between the ages of 18 and 49 ¹. With younger generations expecting a web-based experience for nearly everything from banking to food delivery, APEx ensures your organization is on the cutting edge of patient care that is expected by this generation.

APEx is the dashboard for patient and caregiver and unifies all the other Acurata modules discussed in previous sessions. The Acurata Telehealth module is built seamlessly into APEx allowing video calls with a nurse or other member of your staff. APEx allows a patient or caregiver to request visits directly from the portal, rather than by phone, which saves the time and energy of your staff during peak hours.

Additionally, with text messaging and web chat being the fastest growing and most popular form of communication for Americans under 50 ², we saw the need to provide online chat in APEx for communication between caregiver and/or patient with call center staff.

 

ACCESS TO INFORMATION

Access to information is power and encourages active participation. APEx provides you with the ability to populate a fully customizable knowledgebase where your patients can find resources on their illness, information and resources about your organization, and anything else relevant to their care.

Patients and caregivers also have access to their full history of care as it exists in Acurata. Every visit they have received as well as notes and nurse recommendations can be found easily in APEx.
