Session 2: Referral Management

Welcome back to Acurata 101!

In this session we will learn about Acurata's Referral Management module and how it can help your Admissions Nurses keep track of referrals they receive from referring doctors or other healthcare professionals.

Once the module is activated for your organization, Admissions Nurses can access it from the Menu Ribbon on the top of the screen in the web portal, or on their mobile devices using their mobile application.

 

Capture Prospective Hospice Patients Further Upstream

 

Strategically handling referrals has become more important than ever. A missed appointment or follow up could lead to a lower average stay.

The Referral Management module provides a seamless process that follows a prospective patient from referral, to admission, to triage, while documenting visit activities at each step.

The Referral Management module also includes an automated “tickler file” that allows an Admissions Nurse to use their mobile app to schedule follow up reminders for phone calls, emails, or any other admissions activity that might otherwise slip through the cracks.

Built for the Hospice Industry

 

Acurata provides an easy to use CRM functionality not found in larger, off the shelf, and expensive CRMs. Because Acurata was designed for the hospice industry, you no longer need to try and adapt a generic CRM to reflect hospice processes and terminology. It’s already there for you!

An Admissions Scheduler would simply click on the day an appointment or follow up would be set, and an Admissions Visit window opens. This window allows an Admissions nurse to assign the visit to themselves or anyone else in their team, and provide all the relevant information about the lead.


Mobile Admissions Tracking with the Acurata Mobile App (iOS & Android)

 

Here we can see Jennifer's mobile application screen. Jennifer is an Admissions Nurse and all of her appointments will show up on her mobile app's screen with her appointments displayed by date. She is also alerted when a new Admissions visit is assigned to her, and when she has an upcoming visit.


All the Information a Nurse needs, all in one place

 

Just like in the Triage and Mobile Dispatch modules, Jennifer can tap on any of the Admissions Visit cards to show more information.

All the important details are here, including buttons to start navigation, to voice or video call the patient or caregiver, and the ability to view and/or add notes. Follow up tasks are easily added and displayed on the date the task is due.

 

Accurate Reporting and Documentation

 

Just like reports in the Triage and Mobile Dispatching module, the Referral Management module documents every activity performed by the nurse relating to the admission. Each step is documented by date and time, and by accessing the patient record in the Acurata database, you can see each step and when it happened.
